- UPA waiver
- Medical Release Form
- You can send the waiver to the league at the address listed below, or bring it with you to the first game.
- You must submit a signed UPA waiver (and medical form if you are under 18) or
you will not be allowed to play. No exceptions.
- If you are not a UPA member, you will still have to sign a UPA waiver in order to play.
